No cost involved and people can usually attend as many walks as they wish (no booking required).

Name of scheme: Warwickshire GP Exercise Referral Scheme (PACE)
How can your patients access this service?

Referral by GP or Practice Nurse at Practice signed up to scheme. Scheme operates throughout Warwickshire.

Who is the scheme for? Patients with existing medical conditions who would benefit from regular physical activity in a controlled environment
What happens next? Referrer assesses suitability of patient; completes referral papers and sends to Leisure Centre
Where does it take place?

This is a Warwickshire wide scheme – operating from 13 Leisure Centres across Warwickshire. (Participants are eligible for 24 sessions at a concessionary cost)

Physical Health Enabling Well-Being (PHEW)

How can your patients access this service?

Recommendation by health care professional (mental health) or GP / Practice Nurse. For people living in Rugby Borough.

Who is the scheme for? Patients presenting with mild depression, anxiety or who are recovering from enduring mental health issues who are stable and need no supervision to take part in the activity.
What happens next? Clients present to Rugby Council for Voluntary Service or Rugby MIND with an A5 flyer to book their chosen activity
Where does it take place? At various activity venues within Rugby Borough
